Team Must Provide Waiver Form* for each participant:

* Athletic trainer is not on duty at the tournaments - each player signs the waiver and plays at their own risk.

On Line Payment includes a Convenience Fee

Convenience Fee can not be deducted from the Balance Due if Online Deposit was made

         Example: Fee $325 - Online Deposit Made of $103 - Balance Due is $225

                           Fee $345 - Online Deposit Made of $103 - Balance Due is $245

Balance paid by check should be mailed with the registration form one week prior to the tournament
